This dynamic and highly interactive webinar brings together leading international experts in aging, palliative care, geriatrics, and rehabilitation to explore one powerful question: How can we build an integrated approach that delivers the right care, at the right time, across the entire trajectory for older people living with frailty and cancer? We are organizing this event in collaboration with EU NAVIGATE, a Horizon Europe–funded research project working across six EU countries to adapt, implement, and evaluate a navigation intervention for older people with cancer.

Prof. Caroline Nicholson will set the stage by examining the current realities of care and support for older people living with frailty and cancer. Where are we now — and where are the gaps? - A Framework for Integration
Dr. Rose Miranda will explore how the European Association for Palliative Care (EAPC) White Paper on Frailty provides a blueprint for integrating palliative, geriatric, and rehabilitative care — not only for older people living with frailty, but also for their family carers. - From Theory to Practice
Ms. Rebecca Tiberini will examine, from a rehabilitation perspective, how the integrative model defined in the EAPC White Paper can foster goal-focused, person-centered care in daily practice. - Innovation in Action
Ms. Fien van Campe will present real-world insights from EU NAVIGATE, demonstrating how an integrative palliative, geriatric, and rehabilitative model that constitutes their navigation intervention can support older people living with cancer and their families throughout the entire care trajectory across Europe. - An Expert Panel to Deepen the Conversation
